1. Definition of “ZipStim”
• The term “ZipStim” refers to Dr. Michael J. Passmore and all associated clinicians involved with ZipStim services, as well as ZipStim’s parent company, Encareage Solutions Inc.
2. Eligibility for Services
• Clients must be at least 18 years old.
• All clients are required to complete an initial telehealth consultation to determine eligibility for ZipStim services.
• Clients must disclose all relevant medical history to ensure the safe use of tDCS.
3. Payment and Refundable Deposit
• A refundable deposit equivalent to the full cost of the rented tDCS device is required before the commencement of the rental period.
• The deposit will be refunded upon the successful return of the equipment in good working order, as determined by ZipStim. Any damage or loss will result in deductions from the deposit based on the cost of repair or replacement.
• Full payment for the treatment course, including the refundable deposit, must be made at the time of booking.
4. Equipment Use and Care
• Clients agree to follow all instructions for the safe use of the tDCS device provided by ZipStim.
• The equipment must be kept in a secure and clean environment. Clients are responsible for any damage, loss, or theft of the equipment during the rental period.
5. Course Rescheduling
• Clients may request to reschedule their treatment course with at least 48 hours’ notice before the scheduled start date, subject to availability.
• A rescheduling fee may apply for changes made less than 48 hours before the scheduled start date.
• Courses cannot be rescheduled after they have commenced unless in the case of a verified medical emergency.
6. Refund Policy
• Clients may cancel their treatment course and request a partial or full refund as follows:
• A full refund (excluding the consultation fee) is available for cancellations made at least 72 hours before the scheduled start date.
• A partial refund is available for cancellations made within 72 hours of the start date or after the treatment has commenced, proportional to unused treatment days.
• In the event of a medical emergency preventing the initiation or continuation of the treatment course, a full or partial refund may be granted. A physician’s letter confirming the medical condition must be provided to qualify for this exception.
7. Medical Disclaimer
• tDCS treatment is not a substitute for professional medical advice, diagnosis, or treatment. Clients should consult with their healthcare provider before starting treatment.
• ZipStim is not responsible for any adverse effects resulting from the misuse of the device or failure to follow provided guidelines.
8. Client Responsibilities
• Clients agree to adhere to ZipStim’s protocols for safe and effective treatment.
• Clients are responsible for returning the rented equipment by the specified deadline. Late returns may incur additional charges.
• Clients must report any technical issues with the device immediately to ZipStim for resolution.
9. Privacy and Confidentiality
• ZipStim adheres to applicable privacy laws to protect client information. Personal and medical data will not be shared without client consent except where required by law.
10. Liability Limitation
• ZipStim is not liable for any indirect, incidental, or consequential damages related to the use of its services or equipment.
11. Agreement to Terms
• By booking a treatment course, clients acknowledge and agree to these terms and conditions.
